Please note, sponsorship requires the submission of contact information (name, email address, etc.) for each match participant to be used at Kestrel's discretion. By applying for a sponsorship, you also agree to deliver this information upon approval of sponsorship and permit use of all information collected.

Please complete this form as far in advance of your event as possible. Forms submitted with less than 4 weeks notice will not be considered. We review all submissions on the second Wednesday of each month. Complete 1 form per event.
